By enrolling in a membership with TRAIN ANYTIME, I agree to be legally bound by this Membership Agreement and Waiver of Liability. In exchange for participation in any activities organized by Train Anytime — including access to its facilities, equipment, and services — I, for myself and (if applicable) for members of my family, agree to the following terms:
I understand and acknowledge that participation in physical training programs involves inherent risks. These activities may include, but are not limited to:
I voluntarily choose to participate in these activities and assume full responsibility for any injuries, losses, or damages that may result.
I agree to observe and follow all posted rules, safety notices, and verbal instructions provided by staff, trainers, or instructors affiliated with TRAIN ANYTIME.
I, on behalf of myself, my family members, my heirs, assigns, and legal representatives, hereby release, waive, and discharge:
from any and all liability, claims, demands, or causes of action, present or future, known or unknown, arising out of or relating to my participation in physical training or use of the facility.
By submitting my credit card information, I authorize:
I confirm that I have read, understood, and voluntarily accept this waiver. By submitting the membership form, I acknowledge that I am legally bound by the terms outlined above.
This waiver must be completed by a parent or legal guardian of the minor participant.
By enrolling my child in a membership with TRAIN ANYTIME, I agree to be legally bound by this Membership Agreement and Waiver of Liability. In exchange for my child’s participation in any activities organized by TRAIN ANYTIME — including access to its facilities, equipment, and services — I, on behalf of myself and my minor child, agree to the following terms:

By clicking “I Agree,” I confirm that I am the legal parent or guardian of the participant. I have read, understood, and voluntarily accept this waiver and the associated terms. I acknowledge that I am legally bound by the contents of this agreement.
I hereby agree to accept and be legally bound by this Group Training / Bootcamps Contract. In exchange for participating in physical training organized by TRAIN ANYTIME, its affiliates, and my personal trainer (please provide trainer’s name below), or for use of the property, facilities, and services provided, I agree to the following terms for myself and (if applicable) for members of my family:
1. Rules & Instructions:
I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by class instructors, personal trainers, or staff.
2. Physical Activity Acknowledgment:
I understand that I am enrolling in a program of strenuous physical activity, which may include — but is not limited to — walking, running, jumping, boxing, dance, kickboxing, Pilates, yoga, weightlifting, aerobics, and the use of various conditioning and exercise equipment and facilities. These activities may be designed, offered, recommended, and/or supervised by the instructor and/or TRAIN ANYTIME staff.
I acknowledge that there are certain inherent risks associated with such activities, and I voluntarily assume full responsibility for any personal injury, illness, or property damage resulting from my participation.
3. Waiver & Release of Liability:
I, (please provide your name below), on behalf of myself, my family members, heirs, assigns, agents, officers, directors, employees, and co-workers, hereby release and discharge TRAIN ANYTIME, its instructors, staff, independent contractors, and affiliates from any and all claims, demands, or causes of action for injury, loss, or damage, whether now known or unknown, arising from or related to my participation in the training session.
4. Acknowledgment of Understanding:
I HAVE READ AND FULLY UNDERSTAND THIS WAIVER AND RELEASE. BY SIGNING BELOW, I FREELY AND VOLUNTARILY AGREE TO BE LEGALLY BOUND BY ITS CONTENTS.
